In-Home Care Support Office

An in-home care support office is an office that undertakes comprehensive consultation, planning, liaison, and coordination regarding in-home care for persons requiring long-term care who receive long-term care insurance services. It is also commonly referred to as a care plan center because a chief care manager or care manager is always present and creates care plans (nursing care service plans) that are essential for receiving services.
"Residence" includes not only one's own home, but also rooms in low-cost nursing homes and residential fee-based nursing homes.

In addition, if it is determined that it is difficult to care for the patient at home as a result of assessment and discussions with related parties, we will refer the patient to a long-term care insurance facility (such as a special nursing home for the elderly).
