For first-time patients

Please bring your health insurance card or My Number card (Individual Number Card) with you when you visit for your first visit or the first visit of the month.
If your health insurance policy changes, please present your new insurance card as soon as possible.
If you have claimant certification for medical care (elderly/disabled medical care, etc.), please present it together with your health insurance card.
If you have been prescribed medication at another medical institution, please present your prescription record.
Please be sure to inform us if you are pregnant or may be pregnant.
If you experience any severe symptoms,　please inform reception.
Your consultation will go more smoothly if you provide detailed information on the patient form you fill out in advance.
